Medical Forms
Below are some forms that you may want to download when your child has a doctor, dentist or eye doctor appointment. Please have the doctor complete these forms and return them to me as soon as possible. If, for any reason, your child needs medication at school, there is also a form (located below) that needs to be filled out by their physician.
fHealth History Form: To be completed by the parent at registration or as needed
Promotion of Health: To be completed by the child's doctor. We recommend that students entering the first, fourth and seventh grade, as well as new students, have an exam by a physician. Medication Policy and Authorization: For those extreme cases where it is necessary that medication be administered at school Dental Health: To be completed by the child's dentist. We recommend that students in kindergarten through grade five have a dental screening at least yearly.
Vision Screening: To be completed by the child's doctor upon kindergarten entry and as needed.
